Physical Therapist (PT) – Per Diem Company: Caringhand Home Health Solutions, Inc. Position Type: Per Diem Work Arrangement: Field-Based – Patient Homes Compensation: $160–$180 per visit About the Position Caringhand Home Health Solutions, Inc. is seeking a compassionate and qualified Physical Therapist (PT) to provide home health physical therapy services to patients in California. The ideal candidate is self-directed, organized, and committed to helping patients improve their mobility, functional independence, and quality of life. Key Responsibilities Assess patients’ functional abilities, mobility, muscle strength, gait, range of motion, home environment, and rehabilitation potential. Develop and implement individualized physical therapy treatment plans in accordance with physician orders and applicable California regulations. Provide therapeutic interventions and educate patients and caregivers on treatment goals, exercises, and assistive devices. Monitor patient progress and responses to treatment, reporting significant changes in condition to physicians. Coordinate care with physicians and other healthcare professionals and recommend additional home health services when appropriate. Maintain accurate clinical documentation, progress reports, and discharge plans. Supervise Physical Therapy Assistants in accordance with applicable regulations and organizational policies. Provide caregiver guidance, staff education, and other related services as needed. Important Position Details: This is a field-based, per diem position requiring travel to patients’ homes. Although some job platforms may categorize the position as remote, this is not a work-from-home or telehealth role.
