Days: Mon, Tue, Wed, Thu, Fri. LPN Home Health. Home Office: Hartford HealthCare at Home, 81 Meriden Avenue, Southington, CT 06489. Towns: Middlefield, Middletown, Durham, Haddam. Killingworth, Chester, Deep River and surrounding towns Schedule: M-F, 8 hour days. On Call Requirement(s): No On-Call. Experience Needed: Home Health. Include Skills Checklist with Submittals: Looking for experience with wound vac, drains, foleys - mainly to ensure they assign or don't assign specific patients. EMR: HCHB and PointCare.
